Regular review and assessment of a person with type 2 diabetes involves:

1. SUPPORT HEALTHY BEHAVIOUR INTERVENTIONS • Reinforce and support healthy behaviour interventions and lifestyle changes expected to reduce blood glucose levels 2. ASSESS FOR “CHANGE IN CLINICAL STATUS”, i.e. HIGH-RISK POPULATIONS High-risk populations include individuals: • With atherosclerotic cardiovascular disease • with a history of heart failure (with or without reduced ejection fraction) • with chronic kidney disease (CKD), or • ≥60 years of age with ≥2 cardiovascular risk factors Atherosclerotic cardiovascular disease (ASCVD) can be defined as a person who has • Had a myocardial infarct (heart attack) and/or angioplasty • Angina (chest pain on exertion) • Peripheral arterial disease (pain on walking) • Had surgery for blocked blood vessels (arteries) in the legs • Had a cerebrovascular accident or transient ischemic attack (stroke or TIA) • Had surgery to treat carotid artery blockage
