iv. Timely adjustments to antihyperglycemic therapy should be made to attain A1C targets within three to six months (24).

v. Ongoing management of hyperglycemia (BG targets are not being met): • reinforce healthy behaviour interventions and lifestyle choices expected to lower blood glucose levels • Add another drug from a different class or titrate the dose of the existing antihyperglycemic agent(s); • Add bedtime basal insulin (see figure 3); OR • Intensify the insulin regimen.
