Summary of program development so far:
Step I: Define/assess/recognize needs • Explore the problem or issue with the patients or the population. identify issues, problems amenable to change. o Educator goal : Inform professional actions relating to program design for individuals, groups and/or populations. o Individual goal: Personal recognition/acceptance of problem and need to change behavior. Step II: Develop plan for learning/intervention • Clarify feelings and the meaning of the issue to the person. o Educator goal: Design/implement a program to address this need. • Problem-solve collaboratively to develop a change plan. o Educator goal: Use knowledge of change processes and diabetes self-care to collaborate on/design the best plan for the individual or group. • Set goals collaboratively. o Educator goal: Use knowledge of SME and treatment/care options to help the person understand and select/define learning objectives that will likely result in success. Step lll: Pilot, revise, implement • Commit to action. • Educator goal : Pilot the delivery of the program. • Evaluate delivery, revise, implement.
