Nutrition This part of the assessment includes information about the individual’s pattern of food consumption and fluid intake relative to metabolic requirements.
Data collection should include:
Rationale for including this assessment area & application to SME: • BMI calculation, waist circumference and related risk. • Assessment of cardiovascular risk and need for heart health education. • Evaluate the person’s understanding of the relationship between food intake, medication and BG. • Concurrent medical conditions that have nutritional implications, such as weight problems.
Clinical data: • Height and weight • Weight history • BMI • Waist circumference • Body frame
• Blood pressure • Family history • Laboratory data: BG, lipids, A1C, any abnormal findings • SMBG Diet history: • Usual food intake determined by type, form and amount of food; frequency of meals and snacks; use of food records, 24-hour recall or frequency form. • Adequacy of intake to meet individual nutritional requirements and appropriateness for management of diabetes and other conditions, such as dyslipidemia and obesity. • Attitudes toward, and knowledge
• Changes in food choices and behaviours that are acceptable to patient and of benefit for daily control. • Evaluate type of educational approach needed, learning style. • Knowledge gaps, stage of change toward different food behaviours, indicators of eating disorders. • Calculation of daily nutrient intake: o Energy intake o Nutritional adequacy o Distribution of nutrients o Carbohydrates, protein, fat types
