Conclusion Assessment lays the foundation for all future interactions with patients. Obtaining information in all areas of health care enhances the opportunities for holistic treatment of the person with diabetes and his or her family. However, only a trusting, therapeutic relationship between you and the person/family will allow you and the person to evaluate options and make appropriate health-care choices in a collaborative, empowering environment. Assessment is a complex process that may well occur over a long period of time as the relationship develops between you and your patient. There are excellent tools available on the new Diabetes Canada 2018 Clinical Practice Guidelines website to facilitate your initial assessment and to promote the continual flow and sharing of information.
