Degludec U100 or U200 (Tresiba) • Insulin degludec U100 and U200 form stable, multihexamer chains which are slow and consistent to dissociate. This results in a protracted time action profile due to the delayed absorption from subcutaneous tissue depots into the systemic circulation (77). It has an onset of 90 minutes (78). • The duration of action is 42 hours (78). • Steady state is reached in approximately three to four days (86). • Once daily injection: o Can be given at any time of day. A minimum of eight hours between injections is required (87). • Switching to degludec o Doses of meal insulin and corrections may need to be adjusted. o People with type 1 diabetes: ▪ Reduce the degludec dose by 20% to lower the risk of hypoglycemia (86). o People with type 2 diabetes: ▪ Once-daily long or intermediate-acting insulin: Can be done unit-to-unit (86). ▪ Twice daily long- or intermediate-acting insulin, reduce the dose by 20% to lower the risk of hypoglycemia (86).
